Job description

Your role in our mission

We are seeking a Healthcare Assistant to join our multidisciplinary community team in the West Midlands. In this role, you will provide holistic, person-centred end-of-life care and support to patients in their own homes, delivering compassionate and high-quality care when it matters most

Your duties will include monitoring and maintaining patients' skin integrity and personal hygiene, supporting their nutritional needs, ensuring they get adequate rest and sleep, and assisting with mobility. You will also help keep the care environment clean and organised and provide essential support in symptom management.

You will demonstrate empathetic and professional communication with patients and their significant others, consistently providing high-quality personal care and support to enhance overall wellbeing.

Contract: Permanent
Working pattern: Monday to Sunday, including bank holidays
Shift pattern: Shifts rostered between 7:30am and 8:00pm
Salary: £25,272 per annum
Base: Community-based across the West Midlands, with a hospice base. The postholder will primarily provide care in peoples’ own homes, with occasional support on the inpatient unit.

What we are looking for: 

  • Recent experience providing care within a health or social care setting, including an understanding of palliative and end-of-life care.
  • Experience of delivering care in a community setting and contributing to person-centred assessments.
  • NVQ Level 2 in Health and Social Care, or an equivalent Level 2 qualification, is desirable but not essential.
  • Person-centred and compassionate when caring for the person and those important to them. 
  • Ability to develop and evaluate care management plans that meet the complex needs of the person, and those important to them. 
  • Emotional resilience and the capacity to work independently.
  • Possession of valid driving license, access to a car, and be willing to drive up to 25 miles.
