Essential Job Duties

• Conduct Social Work assessments to determine the psychosocial needs, preferences, and goals of the participants.
• Actively participate in IDT meetings to develop participant care plans based on assessment findings.
• Deliver and document social work interventions as outlined in participants' care plans, including arranging necessary resources and services, providing counseling, and offering case management.
• Collaborate with the primary care physician and other members of the care team to ensure smooth care transitions between settings.
• Initiate, coordinate, and facilitate care conference meetings to enhance care coordination among care team members, participants, and their support network.
• Provide discharge planning for participants disenrolling from the program.

Job Requirements

• Masters Degree in Social Work (MSW) required.
• Minimum of one (1) year of experience with a frail or elderly population.
• Previous experience coordinating and facilitating care conference meetings.
• Preferred previous experience assisting individuals with behavioral health and substance abuse issues.
