Pay range for this position is $34.25 - $51.13/hour. Actual pay is based on years of licensure. This is a Salaried position.

Overview

POSITION SUMMARY: The Wound, Ostomy, Continence (WOCN) Coordinator is responsible for coordinating wound, ostomy and continence care in their assigned area. It is the role of the Coordinator to collaborate with regional clinicians to oversee the care of wound/ ostomy and catheter patients. Completes home visits as needed to meet the needs of patients, provide education/ clinical competency evaluation and oversight of care. Acts as the patient advocate, maintaining collaboration with the patient’s care team, providers, patients, caregivers, community agencies, medical supply vendors, and leadership. Oversees wound, ostomy and continence related clinical outcomes and quality measures in their assigned region by completing/ reviewing clinical reporting and auditing. Participates in quality improvement activities.

Qualifications

REQUIRED QUALIFICATIONS

Education: BSN

Experience: 1 year experience as WOCN in any care setting

Licensure/Certifications Registered Nurse in current state

Certified Wound, Ostomy, Continence Nurse (CWOCN)

Valid Driver's License

Own transportation and auto insurance.

CPR through American Heart Association (BLS for Health Care Providers)

Other skills or knowledge Exceptional customer service and phone etiquette

Basic computer skills with Microsoft Office

PREFERRED QUALIFICATIONS

Experience: 1 year clinical experience in Home Care Setting

Other skills/knowledge: Working knowledge of EMR
