As we continue to grow, we are looking for a Social Navigator to join our team at our Church Street location in downtown Edmonton and help us deliver the Mint experience — thoughtful service, strong collaboration, and care that puts people first.

About the Ace Team

The Adherence and Community Engagement (ACE) Team is a multidisciplinary outreach team dedicated to engaging hard to reach people experiencing health and social inequity. The ACE Team manages 2 teams with each having a complex caseload of 25-35 clients, operating 7 days a week, 365 days a year to provide wrap around health and social supports. We recognize that a client’s health outcomes and their ability to address their basic needs, goes hand in hand. ACE is a dedicated group of pharmacists, nurses, outreach, and social workers who go above and beyond everyday for their clients.

Responsibilities of the Social Navigator

  • Work collaboratively as part of the ACE Team to connect clients to social supports.
  • Creative problem solving for client social needs including food access, funding supports, emergency pet care, family supports, furniture, counseling, and supportive resources.
  • Complete required housing assessments or referrals such as the Service Prioritization Decision Assistance Tool (SPDAT) and provincial assessments.
  • Navigate and support clients in accessing appropriate income funding available as well as health benefits.
  • Strong ownership of care planning as it relates to social supports, follow through and follow up.
  • Effectively delegate follow up tasks to other team members to ensure continued progress during off-shift days.
  • Efficiently document client care notes on Med Access EMR.
  • Educate other ACE Team members of existing gaps, new resources, or alternative ways to improve client care.
  • Engage, communicate, and advocate with a variety of health and social audiences to improve existing systemic barriers.
  • Comfort driving, getting in and out of a vehicle and entering homes, shelters, community drop in spaces to see about 30 clients daily.

Education and Certifications

  • Diploma in Social Work is required.
  • Police Information Check with Vulnerable Sector.
  • Active Alberta Class 5 Driver’s License, clean 3-year Drivers Abstract.
  • Certifications (or willingness to obtain): Standard First Aid and CPR.
  • Training in Trauma Informed Care, Cultural Sensitivity, Stigma, and/or Harm Reduction.

Experience

  • Minimum 6 months of experience working with marginalized, high risk and vulnerable populations.
  • Working knowledge of harm reduction practices, abuse and trauma, sexual exploitation, addictions, mental health, homelessness, inner city, and Indigenous culture.
  • Knowledge of and experience with Motivational Interviewing and using evidence-based models of care.
  • Expert knowledge of Edmonton community resources for housing, funding, ID, shelter, and basic needs.
  • Demonstrated experience de-escalating crisis and potentially violent situations with individuals under the influence of drugs or alcohol, under stress &/or struggling with acute/chronic mental health crises.
  • Demonstrated experience completing housing referrals/applications, AISH or other funding applications.
  • Demonstrated ability to collaborate with various social and healthcare disciplines to deliver person centered care.
  • Comfortable using mobile technology- smartphones and phone applications, mobile laptops, Google Drive, Google Calendar, electronic platforms for client documentation.
