Job description

We are working closely with a local authority in L iverpool to assist with the appointment of a Reablement Assistant , on a 1 3 -months contract, highly likely to be extended at clients discretion. Please apply with your CV for immediate consideration.

Summary

The Reablement Assistant plays a key role in supporting adults to maximise their independence, wellbeing and quality of life through person-centred reablement and residential support services. Working within Sedgemoor Hub, a 30-bed intermediate care, dementia and respite service, the postholder assists individuals to regain, maintain or improve their daily living skills while promoting dignity, choice and independence. Working collaboratively with families, health professionals, social care colleagues and partner agencies, the Reablement Assistant delivers high-quality practical, emotional and personal care support in accordance with individual support plans. The role contributes to safe hospital discharge, prevention of unnecessary admissions to long-term care and improved outcomes for adults requiring reablement, dementia care and respite services.

Responsibilities

  • Support individuals in achieving outcomes identified within their care and support plans.
  • Provide personal care support including washing, dressing, toileting, grooming and mobility assistance in accordance with care plans.
  • Support individuals to develop practical life skills required for independent or assisted living.
  • Assist with activities including hydrotherapy, rebound therapy and other rehabilitation programmes where appropriate.
  • Support residents receiving respite care and longer-term residential care.
  • Recognise and report safeguarding concerns in accordance with Council policies and statutory guidance.
  • Report significant changes in service users' health, wellbeing or circumstances to the appropriate manager.
  • Comply with Liverpool City Council policies, health and safety legislation and professional standards.

Qualifications

  • NVQ Level 3 in Health and Social Care or willingness to work towards achieving the qualification.

Essentials

  • Experience working within an Adult Social Care, residential care, community care or reablement setting.
  • Understanding of person-centred care and reablement principles.
  • Knowledge of adult safeguarding legislation and best practice.
  • Understanding of dignity, choice, independence and strengths-based approaches.

  • You should be available to work immediately or at a short notice.
  • You should have right to work in U.K
  • This role requires an Enhanced DBS
